Telehealth Consent & Information telehealth-consent
Important information about your video visits.
Telehealth lets you receive care from Daystar Family Care by secure video. Before your first video visit, it helps to understand how it works and its limits.
What to expect
Your provider will see and speak with you over a secure connection, review your concern, and recommend a plan — which may include prescriptions, lab orders, a referral, or an in-person visit. Telehealth is available to patients located in Ohio at the time of the visit.
Benefits & limitations
Telehealth is convenient and protects your privacy, but it has limits: some conditions require a hands-on exam or testing. If your provider determines you need to be seen in person, they will tell you and help you schedule. In an emergency, call 911.
Your consent
By joining a telehealth visit, you consent to receive care by video and understand the information above. You may stop a telehealth visit at any time and request an in-person appointment instead. Standard visit fees and insurance rules apply.
