We use and share your health information to provide your treatment, coordinate care with other providers you see, arrange payment for services, and run our practice operations. For most other uses, we will ask for your written permission first.

- Ask us to correct information you believe is incorrect.
- Request confidential communications and ask us to limit what we share.
- Get a list of those with whom we've shared your information.
- File a complaint if you feel your rights have been violated.
Our responsibilities
We are required by law to maintain the privacy and security of your protected health information and to notify you promptly if a breach occurs that may have compromised it. We will not use or share your information other than as described here unless you tell us we can in writing.
